Qualifications:
A bachelor's degree from an accredited institution and one year of relevant experience working in a health care setting, a NH license as an RN, with at least one year of relevant experience working in a health care setting, an associate's degree from an accredited institution plus two years of relevant experience working in a health care setting, a NH license as an LPN, with at least two years of relevant experience working in a health care setting or be a high school graduate or have a GED with six years of relevant experience working in a health care setting with at least two of those years as direct care personnel in a long term care setting within the last five years.
Ensuring Rose Meadow complies with all applicable federal, state and local laws and regulations.
Ensuring a safe environment for residents and personnel.
Uphold NH Patient's Bill of Rights and perform all duties in accordance with Rose Meadow's Mission Statement.
Participate in the generating of, and ensure compliance with, all resident care plans and service agreements, if applicable.
Ensure the delegated duties, i.
e.
monthly progress notes and 90-day care plan progress notes, ordering of supplies, resident annual wellness visits, resident chart compliance, etc.
are up to date, complete and timely to comply with facility policy and state regulations.
Review and sign off on all monthly progress notes and 90-day care plan progress notes.
Complete admission paperwork with all new residents and ensure the new resident and family feel welcomed into the program.
Work cooperatively with the Marketing & Admissions Coordinator, Senior Manager, Client Care Coordinator/RN, and program staff to ensure a smooth transition for a new admission and that the needs of new residents are met upon admission to include; medical equipment, DME supplies, medications/prescriptions, physician orders, a preference profile and a working care plan.
Complete necessary documentation for any employee work-related injury according to Rose Meadow policy/procedure in conjunction with the Worker's Compensation Vendor's requested documentation.
Complete and submit all necessary documentation to the NH Department of Health and Human Services to include but not limited to; Annual license renewal, waiver requests as needed, timely reporting and documentation for Reportable Events, Life Safety Fire Drills and Disaster Trainings, etc.
Oversight and supervision of the Client Care Coordinator/RN.
Communicate and work closely with all Residents, Legal Guardians, Durable Power of Attorney(s) (DPOA), Case Managers and family members to ensure each resident's needs are met by following their case closely at all times.
Shall obtain and document 12 hours of continuing education related to the operation and services each annual licensing period.
The administrator may provide direct resident care.
Demonstrate and ensure professionalism and foster a positive therapeutic environment.
